Completed forms can be returned to Hildebran Hall, Room 102, or you can mail them to Western Piedmont Community College, Financial Aid Office, 1001 Burkemont Ave, Morganton, NC 28655-4504 Students who have been terminated from receiving financial aid may petition the Financial Aid Office to waive the satisfactory progress requirements only where special circumstances are present such as: *Illness to student *Injury to student *Death of an immediate family member *Other extenuative circumstances clearly beyond the control of students which affected their ability to meet academic progress standards. The petition must be accompanied by written, third-party documentation. Childcare Assistance Application Students wishing to apply for childcare assistance should fill out this application and return it to the Financial Aid Office Federal Work Study Application Students interested in participating in the Federal Work Study program must complete this application and return it to the Financial Aid Office. Positions go quickly at the beginning of each academic year so make sure to apply early. Students who report a significantly low income on their FAFSA will be required to complete this form to clarify how they were able to support themselves for the calendar year of 2007.This form helps students explain how they were able to provide for needs such as housing, food, utility bills, etc. Use this form to complete your verification requirements if you are considered a dependent student. If you answer "No" to all of the following questions, you are considered dependent. *Born before January 1, 1985?? *Working on a Masters/Doctorate Program for 2008/2009 school year? *Married as of today?? *Have children whom you provide at least half of their financial support? *Have dependants other than children or spouse who live with you and you provide at least half of their financial support? *Are both of your parents deceased? *Are you currently serving on active duty for the US Armed Forces or are you a veteran of the US Armed Forces? Verification Form (Independent) Use this form to complete your verification requirements if you are considered an independent student. If you answer "Yes" to at least one of the following questions, you are considered independent. *Born before January 1, 1985?? *Working on a Masters/Doctorate Program for 2008/2009 school year? *Married as of today?? *Have children whom you provide at least half of financial support? *Have dependants other than children or spouse who live with you and you provide at least half of their financial support? *Are both of your parents deceased? *Are you currently serving on active duty for the US Armed Forces or are you a veteran of the US Armed Forces? |
